Please note: The Discovery's field trip schedule for the 2011-2012 school year is full. To be added to the waiting list, please complete the form below and we will fill any cancellations on a first come, first served basis.

Field trip option	Note: Reduced cost options are for WCSD public school with 50% or greater free and reduced lunch. The Discovery will verify prior to reservation confirmation.

Desired date	Note: Field trips are available Tuesday through Friday.
Desired time	Note: Please indicate two different time options.
Number of students	Note: Maximum of two classes and/or 60 students per field trip.
Number of chaperones	Note: The Discovery recommends 1 chaperone for every 7 students for elementary groups and 1 chaperone for every 4 students for preschool and kindergarten groups. The Discovery does not charge for chaperones within these guidelines.
